JMSD Policies
By completing this registration and signing below, I acknowledge that I have voluntarily chosen to enroll my child in dance classes at JMSD. In so doing, I agree to release, forever discharge and defend, indemnify and hold harmless JMSD and its officers, employees, instructors and from any and all claims, demands, or causes of action which are in any way connected with my child’s participation in classes and/or programs sponsored by JMSD. I certify that my child is in good health and can participate in all the activities of classes and that I am responsible for notifying my child’s teacher of any physical limitations, illnesses or injuries which may affect his/her participation. After careful consideration of my child’s overall health and the dance instruction program offered at Jami Masters School of Dance, Ltd (JMSD), I acknowledge and understand the inherent risks in this activity and the possibility, despite precautions taken, of accidents or physical injury.

So that our faculty and staff may focus on providing a quality dance program and instruction for your dancer, supervision is not available for any child outside the classroom studios and dancers may only enter a studio when invited to do so by a teacher. To minimize distractions, class disruptions and accidents in the hallways, siblings of dancers may not be left unattended and should remain seated in the lobby or hallway while waiting. Dancers under the age of 8 may not be left unattended by a parent before, after, or between classes. For the safety of all our dancers and staff, please discuss with dancers over 8 the importance of waiting inside the studio for you to pick them up. Your prompt arrival for pick-up and drop-off of your dancer will also greatly assist our staff in keeping class schedules running on-time. Accounts for unattended dancers left over 5 minutes past their class end time may be charged a $15.00 fee.

JMSD reserves the right to use photos and/or videos of JMSD students for the sole purpose of promoting the School in print, video and social media. By initialing below and signing this acknowledgement and release, you acknowledge that JMSD is the exclusive owner of all rights, including copyright, in any photo or video taken, and you irrevocably transfer, assign and otherwise convey to JMSD your entire right, title and interest, if any, in and to all photos or videos taken at the School. If you do not consent to such use, please notify the studio office in writing.

Dancers may add or change classes through January 1, 2020. To request that a class be added, use the ADD/DROP card and please take note that both the teacher and the office staff must confirm availability before the class is added to your dancer’s schedule. To withdraw your dancer from a class, use the ADD/DROP card, include the date and time of submission, and turn it in at the office. Failure to attend class does not constitute a withdrawal and you will still be responsible for payment for the class until 30 days following the date a DROP card is received.

Dancers who must miss a class may attend a similar class on a different day and time. Dancers wishing to make up more than 3 classes in a year and dancers wishing to make up classes after January 1 must consult with office staff to determine class availability for makeups. Instructors are subject to change, with or without prior notice.
